🛡️ Grade 6 Immunization Requirements
To ensure the health and safety of our school community, all students entering Grade 6 must meet New Jersey state immunization requirements. These immunizations help prevent the spread of serious communicable diseases and are mandated by N.J.A.C. 8:57-4.
📌 Required Immunizations for Grade 6 (by age 11):

Tdap (Tetanus, Diphtheria, Pertussis) booster

Meningococcal conjugate vaccine (MCV4)
These immunizations are required once your child turns 11 years old and must be documented before the start of Grade 6, unless a valid exemption is on file.
🔔 Notification Process:

In spring of Grade 5, families will receive a letter from the school nurse outlining the required vaccines and the due date.

If your child has already received the vaccines, please submit updated immunization records to the nurse’s office.

If your child will turn 11 during the school year, they may begin the year on a provisional status but must receive the required immunizations within the grace period allowed by state law.
✅ What Families Need to Do:

Schedule an appointment with your child’s doctor to receive the required vaccines (if not already done).

Submit proof of immunization to the school nurse.

If your child has a medical or religious exemption, you must submit appropriate documentation in accordance with NJ regulations.
⚠️ What Happens If Documentation Is Not Provided:

Students will not be allowed to attend school unless:

The nurse will notify families of non-compliance and outline steps to meet the requirement.

Continued non-compliance will result in exclusion from school until documentation is received.
