Crisis Screening & Safety Planning

When a student shows signs of emotional or behavioral distress, UES follows procedures grounded in NJDOE policy to ensure their safety and well-being. This page outlines how we responsibly assess and support students experiencing crisis.


🔍 Risk Factors That Can Trigger a Crisis Screening

A crisis screening is conducted if a student demonstrates or reports any of the following:

  • Verbal or written expressions of self-harm or harm to others

  • Sudden, intense emotional distress or lack of coping strategies

  • Significant changes in mood, behavior, or social engagement

  • References to trauma or persistent feelings of hopelessness

These behaviors align with NJDOE’s approach to identifying students who may need additional intervention.


đź§­ The Crisis Screening Process

When a concern arises, the following steps are taken:

  1. Initial Consultation
    The student meets with a trained counselor or social worker in a private and supportive environment.

  2. Risk Assessment
    Staff assess the level of risk—low, moderate, or high—using professional judgment aligned with NJDOE recommendations.

  3. Response Based on Risk

    • Low Risk: Student returns to class with a follow-up plan.

    • Moderate/High Risk: The school contacts parents immediately and may refer the student to external mental health professionals or crisis services.


🏥 External Evaluation & Re-entry

If an outside clinical evaluation is deemed necessary:

  • Parents must pick up the student promptly.

  • A professional clearance note is required before the student can return.

  • A re-entry meeting will be scheduled with the school counselor or administrator.

  • Together, a Safety Plan will be created.


🤝 What is a Safety Plan?

A Safety Plan is a support-focused roadmap that addresses:

  • Emotional coping strategies and calming techniques

  • Identified trusted adults in school

  • Targeted school support, such as regular check-ins or schedule adjustments

  • External referrals as needed—e.g., to Care Solace or Mobile Response
