Dear Parent or Guardians of upcoming 6th Grade Students,
Fifth grade students who will be attending school in September will need two immunizations for school.
Tdap: one(1) dose given no earlier than the 10th birthday for those children born on or after January 1, 1997 and in , or entering Grade Six, provided at least 5 years have elapsed from the last DTP/DTap or Td dose. Effective 09/01/08
MENINGOCOCCAL: one (1) dose for those children born on or after January 1, 1997 and in or entering Grade Six. Effective 09/01/08
Please note: These apply to students when they turn 11 years of age and attend Grade Six.
These are required immunizations for school attendance prior to September of sixth grade. If your child has not received these immunizations by September, he or she may be excluded from school until those immunizations are received. Please send a copy of your child’s immunization record to school when they receive these immunizations and their records will be updated at that time. State regulations require the specific dates for each vaccine given. Physician statements or documentation such as child “fully immunized” or “series complete” remain unacceptable under the regulation.
